Scuba Diving Instruction Forms

You should download two of these forms: one medical form, either the medical statement, if you have NO medical issues.
of the physical exam form. WIth the physical exam form, you will need to visit your doctor for a checkup, and
your doctor will have to verify that you are fit for diving.

Most students use the medical statement.

If you are a new open water student, download liability release 1; for all other instruction, including advanced open water
and all specialty courses, download and fill out liability release 2.

(If you have NO medical issues, this is the form to use)

Physical Exam
(if you have medical issues this is the form for you to use. It will require a doctor's approval)

(for open water students)

Liability Release 2
(for advanced open water and specialty courses)
